When a guy is stimulating a woman's breasts (either manually or orally), there are a number of things that might be racing through his mind.
Whilst every guy will probably have his own opinion, here are my thoughts on the subject:

During middle school or high school, guys start noticing that all the girls around them are actually very anatomically-different to themselves.
Breasts are one part of the female anatomy that guys have a natural fixation with. I don't know whether it's psychological, or cultural, or maybe a bit of both... But the female chest certainly does get noticed.
Until a guy has his first sexual encounter, he's likely to spend a significant amount of time wondering and fantasizing about what a breast feels like.
When he finally gets a chance to fondle or stimulate a pair, he might start comparing the reality of the situation to what he'd previously fantasized.
As guys become more sexually mature, their fixation with breasts doesn't necessarily diminish - it just changes.
Touching a girl's 'assets' might not seem as exhilarating, but it will still be enough to trigger all sorts of sexual thoughts.
In my opinion, sexual encounters tend to be heated and passionate. There's often no time to 'think' or 'contemplate'- thoughts just happen naturally and are varied.
