The court house told me they did not have forms for temporary custody. Where can I get forms or what does a affidavitt have to include? What information is needed in a petition? I recently had the mom and I notorize a letter at the bank giving me permission to have temporary legal soul and physical custody. She is currently in rehab and then she may be incarcerated, but the father of the one of the children is already incarcerated. I have no idea where the other father of the other child is. The daughter has never met him I guess he was a one night stand.

It sounds like that should do for now at least. You want to make sure that you have power of attorney that allows you to get medical treatment for them and enroll them in school and receive information from the school about the kids.
It's too bad the court house wasn't more helpful. Try finding a Legal Aid office in your area and they should be able to help you. OR contact your local branch of the Bar Association (google Bar Association and your city or county name) and ask for Pro Bono help (no fee help). They may also be able to help you. Good luck. With everything. And good for you for helping her out and helping out those poor kids. They are going to need a lot of support.
