If you are referring to what is called "Acetone breath" which according to your point of view might be considered "fruity", this can be caused by a form of diabetes (Diabetic Ketoacidosis) or eating mainly protein (or curiously enough, not eating very much of anything.)  This occurs when the body has to metabolise fat to give you energy (because you are low on carbs).
